Structural Shift Towards Indigenous Manufacturing

The Indian medtech industry is undergoing a structural shift towards domestic manufacturing, with Indian companies increasingly developing and manufacturing products that were historically supplied by multinational medical-device companies.

This shift is being supported by increasing localisation of technology, indigenous product development, manufacturing capabilities, cost advantages and the expansion of Indian medtech companies into higher-value product categories.

Companies such as S3V Vascular Technologies, SMT and PolyMed are developing and manufacturing medical devices across areas including cardiovascular, neurovascular, structural heart and other specialised medical-device segments.

Industry Overview

The global cardiovascular devices market was valued at around $24.3 billion in 2024 and is estimated to reach $35.2 billion by 2029, implying a 7.8% CAGR during 2024–29.

Cardiovasular Industry segment

The market can broadly be divided into vascular interventional devices and structural heart devices. Vascular intervention includes coronary, peripheral and neurovascular interventions, while structural heart includes procedures such as TAVR.

Vascular Intervention

The global vascular interventional devices market was approximately $14.2 billion in 2025 and is expected to reach $19.2 billion by 2029. Asia-Pacific accounts for ~58% of the global market, with the regional market estimated at $7.7 billion in 2024 and projected to reach $12.4 billion by 2029, representing a 10% CAGR.

India is among the key markets in the region, with the vascular interventional devices market estimated at $355.5 million in 2024 and projected to reach $505.5 million by 2029, implying a 7.3% CAGR. Growth is supported by increasing cardiovascular disease incidence, an ageing population, expansion of cath labs, rising healthcare expenditure, increasing insurance coverage and the growing presence of domestic medical-device manufacturers.

Coronary Stents

S3V’s existing business is primarily focused on coronary stents, a segment of vascular intervention. The global coronary stent market was valued at approximately $4.56 billion in 2024 and is projected to reach $6.08 billion by 2029.

In India, the coronary stent market was approximately $249 million in 2024 and is expected to reach around $342 million by 2029. The market comprises Drug-Eluting Stents (DES) and Bare Metal Stents (BMS), with growth increasingly driven by the adoption of DES.

India Coronary Stent Market Current Projected (FY30)
Total Market $249 Mn $342 Mn
Drug-Eluting Stents $186 Mn $287 Mn
Bare Metal Stents $62 Mn $55 Mn

INDIAN STENT MARKET KEY PLAYER :

The Indian coronary stent market is dominated by established players such as SMT, Meril Life Sciences, Abbott, Boston Scientific and Medtronic. S3V currently operates at a smaller scale, with SMT having manufactured more than 663,000 stents, highlighting the scale gap with the market leaders

Company Estimated Market Share
SMT 30–32%
Meril Life Sciences 25–30%
Abbott 15–18%
Boston Scientific 8–10%
Medtronic 6–8%
Others 10–15%

Structural Heart Devices

Structural Heart Disease (SHD) refers to conditions affecting the valves, tissues or other structural components of the heart. The global structural heart devices market was valued at approximately $11.1 billion in 2024 and is expected to reach $16.0 billion by 2029, growing at a 7.7% CAGR. Growth is driven by increasing adoption of minimally invasive cardiovascular procedures and greater use of TAVI and other structural heart technologies.

The market includes TAVI, TMVI, occlusion devices and Left Atrial Appendage (LAA) closure devices. TAVI is the largest segment, accounting for around 58% of the global market in 2024, with the market estimated at $6.4 billion in 2024 and projected to reach $8.8 billion by 2029, a 6.5% CAGR.

Indian Structural Heart Devices Market

The Indian structural heart devices market was approximately $39 million in 2024 and is expected to reach around $121 million by FY29. TAVI accounted for more than 60% of the market in 2024, with revenue of approximately $24.5 million, while occluders generated around $8.3 million, representing approximately 21% of the market. LAA closure devices generated around $4.5 million, accounting for approximately 11% of the market and recording the highest CAGR among the major segments at 31.5%.

Financial Performance

S3V’s revenue declined from ₹19 crore in FY23 to ₹13.5 crore in FY25, while gross margin improved from 58.4% to 74.8% during the same period. Despite the improvement in gross margin, EBITDA declined from ₹3.7 crore to -₹0.6 crore, reflecting weaker operating profitability.

The company reported a liquidity ratio of 3.17x, indicating a relatively strong liquidity position. However, ROA stood at only 0.04%, indicating low returns generated on the company’s asset base.

Capacity Expansion & Fund Raise

S3V is establishing a 12-acre Neuro Division facility at the Tamil Nadu Medical Devices Park, Oragadam, Chennai, with an investment of around ₹300 crore. The facility is intended to develop and manufacture an indigenous neurovascular portfolio, including mechanical thrombectomy kits, microcatheters, aspiration catheters, neuro guidewires and stent-retriever systems. The company is also developing TAVR, currently in the pre-clinical stage for a balloon-expandable valve, along with expandable spinal cages, which are yet to be launched.

S3V raised approximately ₹300 crore in March 2024 at ₹176 per share from investors including M. A. Naik and Madhusudan Kela to support its neurovascular expansion. The company is pursuing an indigenous manufacturing model, with key components being developed and manufactured in-house. It aims to reduce the cost of neurovascular products, with the mechanical thrombectomy kit targeted at around ₹50,000 compared with current costs of around ₹4 lakh. This approach is intended to reduce dependence on imported components and enable more cost-effective products for the Indian market.

VALUTION AND PEER TO PEER ANALYSIS :

S3V is currently valued at ₹545 per share, with a Price-to-Book (P/B) ratio of 7.0x. Its valuation is significantly higher on an EV/Revenue basis compared with established global medical-device companies.

Company EV/Revenue
Medtronic 4–5x
Abbott 5–6x
Boston Scientific 6–7x
Edwards Lifesciences 8–10x
S3V 52.8x

S3V’s premium valuation reflects expectations around its high-entry-barrier medtech industry, ongoing development of neurovascular, TAVR and other products, and its focus on indigenous manufacturing. The company’s future valuation will depend on successful product development, regulatory approvals, commercial launches and the ability to scale these new businesses. Government initiatives supporting domestic medical-device manufacturing could also provide a favourable industry environment.
